Dynamic Forms

Dynamic Forms have been deprecated in MyRec.com and are no longer supported.

We now recommend using Custom Forms, which offer enhanced convenience through features such as multiple-choice fields, display elements, physical and digital signatures, direct-link access, and optional email copies of submissions. Custom Form submissions are stored in each member’s account and can include expiration dates, reducing the need to complete the same form multiple times across programs or memberships.

Dynamic Forms in the Cloning Process

Dynamic forms have been deprecated and are not compatible with the cloning process. If you clone an activity or program that has dynamic forms, you will need to create a custom form and attach it to the activities within the program to replace the outdated dynamic forms.
