To view disclaimers that are incomplete for memberships and activities, there is a Disclaimer Completion Report in the Activity Reports and the Membership Reports menu. To view disclaimers that are incomplete for facility rentals, go to the Facility Reports screen and select the type of facility usage to view the disclaimer statuses.

Membership Disclaimer Status

1. Open the Reports menu on the leftside navigation bar
2. Click Membership Reports
3. Select Disclaimer Completion

4. Set any necessary date range, click Search
5. Select the membership to view disclaimer status results
   1. Filter by status
   2. Filter by membership type
   3. View the status column to see if any disclaimers are incomplete

• If there is an Email Disclaimer icon next to the disclaimer, you can click on that to resend
• Results are available from 3/24/2020 and after

Activity Disclaimer Status

1. Open the Reports menu on the leftside navigation bar
2. Click Activity Reports
3. Select Disclaimer Completion

4. Set any necessary date range and make cancellation and billing selections, then click Search
5. or go straight to the activity list and select an activity
   1. Filter by Status
6. View the status column to see if any disclaimers are incomplete

• If there is an Email Disclaimer icon next to the disclaimer, you can click on that to resend
• Results are available from 3/24/2020 and after

Facility Rental Disclaimer Status

1. Open the Reports menu on the leftside navigation bar
2. Click Facility Reports
3. Select the disclaimer option for the type of facility use that applies

4. Set any necessary date range and to include declined requests, if desired, then click Search
5. Select the facility/facility area
   1. Filter by Status

6. View the status column to see if any disclaimers are incomplete

• If there is an Email Disclaimer icon next to the disclaimer, you can click on that to resend
• Results are available from 1/15/2021 and after
